Nottingham Combined Histologic Grade (NCHG) is recommended. (Ellston-Ellis modification of Scarff-Bloom-Richardson (MSBR) grading system) GX Grade cannot be assessed G1 Low combined histologic grade (favorable) G2 Intermediate combined histologic grade (moderately favorable) G3 High combined histologic grade (unfavorable)

Allocation of Histologic Grade

G1 score 3-5. Well differentiated G2 score 6-7. Moderately differentiated G3 score 8-9. Poorly differentiated

Summary of semi-quantitative method for assessing histologic grade

Tubule Formation (% of Carcinoma Composed of Tubular Structures Score >75% 1 10-75% 2 ?less than 10% 3 Nuclear Pleomorphism (Nuclear grade) Score Small, uniform cells 1 Moderate increase in size and variation 2 Marked variation 3 Mitotic Counts / 10HPF Score Dependent on microscopic field area 1~3 * Histologic grade는 G1에서 G3로 갈수록 나쁜 grade를 의미함. * Nuclear grade는 NCHC (MSBR) 중 nuclear pleomorphism을 가리키는 것으로서, high score (G3)일수록 더 나쁜 grade이며 irregular cells distorted in size를 보임. Black’s nuclear grading system은 역으로 G3에서 G1으로 갈수록 더 나쁜 grade를 나타내며 잘 사용하지 않으며, 대신 G1->G3순서로 된 Modified Black’s nuclear grading system을 이용. * Histoloigc grade / Nuclear grade는 각 병원의 병리학적 결과에 따라 입력.
